Dues waivers are granted for one membership year and are based on the number of years of paid membership. If you receive a dues waiver, all Society membership benefits will continue for that year. This form may also be used to reapply in subsequent years. Permanently disabled members will need to provide a physician's note but need not reapply annually if approved.
